Cold sore or pimple? How to tell the difference, and why it matters
A small, sore bump near your lip could be a pimple or the start of a cold sore, and in the first day they can look alike. It matters which, because the treatments are different, and a cold sore is contagious while a pimple is not.
A cold sore usually starts with tingling, itching or burning before anything shows, then becomes a cluster of small fluid-filled blisters on or right at the edge of the lip, often in the same spot each time. It weeps, crusts and heals over 7 to 10 days, and it is contagious. A pimple is a single tender bump, usually with one white or yellow head, around a pore on the skin rather than on the lip itself, with no warning tingle. It is not contagious and usually settles within days.

The three clues that settle it
The tingle. Most people with cold sores feel something first: tingling, itching, burning or tightness at a spot on the lip, hours before anything is visible. Pimples do not do this. More on that warning stage in the cold sore prodrome.
The shape. A cold sore is several tiny blisters grouped together, which may merge. A pimple is one bump with one head.
The place. A sore right on the red edge of the lip, especially one that has appeared in that exact spot before, is much more likely to be a cold sore. Pimples form around pores, which the lip itself does not have.
Why the difference matters
- Treatment is different. Spot treatments for acne can irritate a cold sore, and antiviral cream does nothing for a pimple
- Timing matters for cold sores. Antiviral treatment works best at the very start, so recognising it early helps
- Cold sores are contagious. Avoid kissing, sharing cups, towels or lip products, and keep away from newborn babies until it has healed
- Squeezing a cold sore spreads the virus and slows healing
Did I pop a pimple or a cold sore?
If you squeezed it and clear or yellowish fluid came out of several small points, and it is on or at the edge of your lip, it was probably a cold sore. Wash your hands straight away, avoid touching your eyes, do not share towels or cups, and leave it alone from now on. It will likely weep and then crust over the next few days. Are cold sores an STD?
Cold sores are caused by the herpes simplex virus, most often type 1, which most people catch in childhood through ordinary contact such as a kiss from a relative. They are not usually a sign of a sexually transmitted infection. The virus can, however, be passed to the genitals through oral sex while a sore is present, which is one more reason to wait until it has fully healed.
When to see a doctor
See a doctor or pharmacist if you are not sure what it is, if it is not healing after two weeks, if sores keep coming back often, or if you have a weakened immune system.
